Physical Examination

Students registering for Athletics must have a current physical examination on file in the Nurses office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and returned to East Orange Campus High.

The 2 Step Process

Step 1- Complete the full athletic packet Click Here (WINTER Sports only at this time):

             -Be sure to fill out all sections and acknowledge all forms.

             - Not having a complete registration in which all forms are complete will delay the process to be cleared.

Step 2- Return completed sport packet to your school nurse. Each sports physical is only valid for 365 days in New Jersey.   Please work to procure a Doctor’s Appointment to complete your child’s sports physical paperwork (Pre-Participation Evaluation Forms-PPE).

ALL HIGH SCHOOL PRE-PARTICIPATION PHYSICAL EXAMS AND HEALTH HISTORY QUESTIONNAIRES MUST BE RETURNED TO:
